DIY PIC 12C509 adapters for the S4

I once found a description of the connections needed for these 8 pin devices on an S4 - but I'll be darned if I can find it now

I only need to program a couple of these devices so the purchase of an adapter is rather OTT.

Can anyone supply the connections needed ?
